These questions are aimed at making sure you realise the extra responsibilities that come with owning a dog, to help you determine what type of dog you are looking for, and to assist us in ensuring the right match for each animal in our care, based on their personality and requirements. For example, if the dog you are seeking is to be a 24 hour inside dog then an outside active dog would not be appropriate.

Please ensure you include all relevant information in this application. Only that information which is provided in this application will be considered in assessing suitability of your chosen dog(s).

Will the dog have regular contact with children 5 years or under (own children, grandchildren, visiting children) or elderly people?

Why do you want to adopt a dog?

Do you currently have any pets? Please give breed, size, sex, age and whether your dog is desexed.

Where do your current pets sleep? Where will your new dog sleep? Please be specific.

Which areas of the house/yard will the dog have access to? (e.g. backyard only, backyard and laundry, kitchen, non-carpeted, other areas). Do you have a doggie door?

Will the dog be allowed inside? How often, eg never, in evening, when somebody home, all the time? What areas will be off limits?

Have you had any pets in the past? What happened to them? Have you ever taken an animal to a shelter or released it to another party?

Is your yard securely fenced? Describe type of fencing (wooden, colourbond, wire etc) and height at the lowest point. If you have a swimming pool is it separately fenced?

If no-one is at home, where will the dog stay? Inside or Outside? If the dog is outside, what type of shelter will the dog have?

How many hours will the dog be without human company for each day?

How often will you have time to walk the dog? How long for? On lead or off lead?

Under what circumstance would you give up this pet?

If you are renting, please provide us with your landlords name and phone number for confirmation that a dog is permitted:

Do you currently have a vet? If so, please provide us their name and phone number so we can contact them for a reference. If you do not have a vet please provide two personal references. We reserve the right to contact these referees should the application proceed.

Is there anything else you would like to tell us about you, your family, or your interest in adopting a dog? The more information you provide, the better we will be able to determine suitability of your chosen dog(s).

By submitting this application, I agree that all the information I have provided is correct. Providing untruthful answers or failure to comply with the requirements of this application or the signed adoption contract can result in the forfeiture of the adoption. You agree to allow us to check the references provided. We will require the successful applicant to sign an adoption agreement and where appropriate allow a house/yard check before adoption.

We may also ask to sight your driver’s license before the application proceeds.
